Responsibilities:

• Assess and coordinate patient admissions, discharges, and transfers in a timely and efficient manner

• Communicate effectively with patients, families, and healthcare providers to gather necessary information and address concerns

• Collaborate with interdisciplinary teams to develop and implement individualized care plans

• Maintain accurate records and documentation to ensure compliance with regulatory requirements

• Provide exceptional customer service to patients, families, and staff, demonstrating a commitment to our organization's values

About the Role:

As an Admissions Concierge, you will have the opportunity to make a lasting impact on the lives of our patients and their families. You will work closely with our healthcare team to ensure a smooth transition from hospital to home and provide ongoing support to patients and families throughout their care journey.

What We're Looking For:

• 1 years of experience in skilled nursing administration or a related field

• High school diploma or equivalent required; degree in healthcare administration, business, or a related field preferred

• Excellent communication and organizational skills with the ability to work effectively in a fast-paced environment

• Empathy and compassion for patients and families, with a commitment to providing exceptional customer service

• Ability to maintain confidentiality and handle sensitive information with discretion
