Role Description
This is a full-time, on-site position located in Sacramento, CA, near Northgate Blvd.
The Community Health Worker will engage with individuals and families to promote health and well-being within the community. Responsibilities include providing health education, connecting clients to healthcare and social services, advocating for community needs, and supporting efforts to address health disparities. The Community Health Worker will collaborate with healthcare teams and community organizations to strengthen the community’s access to resources and improve health outcomes.
Qualifications
- Strong communication, active listening, and interpersonal skills to effectively engage with diverse populations
- Knowledge of community resources, healthcare systems, and data collection methods
- Proficiency in outreach coordination, organizing community events, and delivering health education programs
- Ability to build trust, maintain confidentiality, and advocate for individuals and community needs
- Proficiency in basic technology tools, including email, documentation software, and scheduling systems
- Experience in community health or a related field; familiarity with social determinants of health is a plus
- High school diploma or equivalent required; additional training or certifications in healthcare or community health work are highly desirable
- Bilingual abilities are an asset for supporting diverse populations
