What are the responsibilities and job description for the CAT Behavioral Health Counselor/Case Manager position at Peace River Center?

The staff member of the Community Action Treatment (CAT) Team functions as a clinical member of the multidisciplinary team using clinical skills and experience in assessing, planning, developing, coordinating, and providing treatment, rehabilitation, and support services to program participants and families with severe and persistent mental illnesses under the clinical supervision of the Program Manager and the Psychiatrist/ARNP. The staff member will be responsible for providing clinical interventions including group therapy as well as supportive individual and family counseling. In addition, this staff member will provide case management support through assisting recipients in gaining access to needed medical, social, educational, and other services. The primary goal is to provide supportive counseling while optimizing the functioning of recipients who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. The DSM is to be utilized to determine the mental health diagnosis of the child and therapeutic/clinical interventions needed. This is a hybrid position combing both suppportive counseling as well as case management support. Services are primarily community-based, occurring in the home and school setting but may also take place in the office pending upon specific client need. This position will serve the Tri-County area of Polk, Highlands, and Hardee Counties.

  • Maintain appropriate contacts with community providers and agencies who may be working with the client to support continuity of care;
  • Maintain accurate client records, completing all required documentation and assessments in Credible in a timely manner as mandated in the CAT Team policies and procedures.
  • Provide supportive counseling in an individual, family and group format including coordinating and monitoring the activities of the individual treatment team;
  • Conduct appropriate assessments/interventions which include assessment of substance abuse/co-occurring issues, mental health disorders, and family systems issues;
  • Provide case management support through assessing, linking, monitoring, advocating, and planning for client needs as well as assisting the client and family in gaining access to needed referrals and resources to strengthen overall functioning, including but not limited to medical, social, educational, and other services.
Qualifications:
Education:
  • Master’s degree in one of the core Behavioral Health Sciences or actively enrolled in an equivalent Master’s program.
Experience:
  • At least two years of experience in the mental health field. With an infield degree, at least one year of experience working with children and adolescents with serious emotional disturbances (may include Internship and Volunteer Services).
  • Must have knowledge and experience in using local community resources as referrals
  • Experience in basic child development
  • Experience in family relations
Certifications:
  • Professional enhancement training annually including all required PRC training
Training:
  • Complete all PRC required trainings
Knowledge and Skills:
  • Strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and servicesComplete all PRC required trainings
  • Ability to work independently with minimal supervision
  • Ability to work with Team approach
  • Skills and competence to establish supportive trusting relationships with persons with severe and persistent mental illnesses and respect for person’s rights and personal preferences in treatment
  • Good written and verbal communication skills
  • Computer skills: Microsoft Office, Credible and Intranet, EHR
Safety Equipment:

  • Comply with Occupational Safety and Health Administration (OSHA) Rules and Regulations
  • Life Safety Equipment (Fire Extinguisher)
Transportation
  • Must have a valid Florida driver’s license and clean driving record
  • Must carry and maintain $50,000/$100,000 bodily injury coverage on personal vehicle and provide proof of coverage

Physical Requirements:
  • Ability to lift 30 pounds, climb stairs, and ambulate safely throughout the community

Technology Requirement:
As an employee of Peace River Center, if you are not assigned a company-issued phone, you will be required to authorize the use of the Microsoft Authenticator app on your personal mobile device. This is necessary to access our payroll and communication system, Paycor, and Microsoft email as examples. This will ensure secure and efficient communication with the organization.
Work Environment:
This is a fast-paced work environment including office, field, or community settings. The populations cared for will include patients who might be confused, delusional, irrational, agitated, or uncooperative. The noise level in the work environment is usually moderate. The CAT Team Therapist must understand and accept the possibility of exposure to inside environmental conditions such as communicable diseases and physical/verbal abuse from an out of control patient/client.

Salary : $50,000 - $100,000
