Care Coordinator Non- Targeted Case Manager/Lakeland, FL/1408/SE

The Care Coordinator Non-Targeted Case Manager is responsible for coordination of services, supports, and cross system collaboration for recipients identified as High Need/High Utilizer (HNHU). The Care Coordinator facilitates continuity of treatment for recipients while in psychiatric treatment and assists the recipients in accessing the service delivery system subsequent to their discharge. Care Coordination is time-limited with a heavy concentration on educating and empowering the person served. The Care Coordinator is responsible for utilizing SOAR (SSI/SSDI Outreach, Access, and Recovery) to assist recipients in applying for Social Security Benefits.

Essential Duties/Requirements/Activities:

·Provide timely, accurate, completion of required documents

·Complete SOAR certification within three months of hire

·Represent the agency in multiple committees and conference calls as it relates to Care Coordination

·Participate in daily CSU staff morning meetings

·Provide transportation to participants for scheduled medical, outpatient, dental, social security office, etc.

·Attend monthly Care Coordination Committee Meeting

·Daily review the census of both CSUs for any High Need/High Utilizer (HNHU) clients

·Complete Care Coordination Monthly Report to DCF

·Manage caseload of 20 HNHU clients and report case list to Central Florida Behavioral Health Network monthly

·Participating in quarterly Polk Care Coordination Meeting

·Provides discharge planning and linkages to HNHU clients at the CSUs. Provides time limited support to clients after their discharge to ensure individual's needs are met.

Qualifications:

Education:

Bachelor's Degree in a Human Services field from an Accredited College or University

Experience:

Experience preferred but not required

Training:

Case Management and SOAR Training, completed with supervisor upon hiring

Knowledge and Skills:

Assessment

Excellent written and verbal communication skills

Highly organized with the ability to work with minimal supervision

Excellent skills and knowledge of Microsoft Office

Transportation:

Must have reliable transportation and a FL driver's license in good standing

Must carry $25,000/$50,000 bodily injury coverage on personal vehicle and provide proof of coverage

Must be able to drive own vehicle

Must be 25 years of age and have a clean driving record in order to drive PRC vehicles

Technology Requirement:

As an employee of Peace River Center, if you are not assigned a company-issued phone, you will be required to authorize the use of the Microsoft Authenticator app on your personal mobile device. This is necessary to access our payroll and communication system, Datis, and Microsoft email, as examples.This will ensure secure and efficient communication with the organization.

Work Environment:

The work environment is fast-paced and consists of exposure to physical conditions typical of a normal office environment. The populations cared for will include patients who are confused, delusional, irrational, agitated, or uncooperative. Most of the work is performed while sitting, although the work may require occasional standing or walking and/or the lifting and carrying of small objects. A small amount of travel may be required for meetings, etc., to another PRC location.
