The Opportunity:
We are seeking a skilled, compassionate, and independent Registered Nurse (RN) to join our growing team. As a home health RN at Peace & Petals, you will be a vital member of our caregiving team, providing skilled nursing services, managing patient care plans, and acting as a liaison between clients, their families, and physicians. This role is perfect for an RN who thrives in an autonomous environment and is dedicated to building strong, personal relationships with their patients.
Key Responsibilities:
- Conduct comprehensive in-home patient assessments and develop personalized plans of care in collaboration with the client's physician.
- Administer skilled nursing care, including but not limited to, wound care, medication administration, injections, and IV therapy.
- Monitor and document the patient's condition, vital signs, and progress, reporting any significant changes to the physician.
- Educate patients and their families on disease management, medications, self-care techniques, and symptom recognition.
- Coordinate care with other healthcare professionals, such as physical therapists, social workers, and CNAs, to ensure a holistic approach.
- Provide compassionate emotional support and guidance to patients and their families.
- Maintain accurate, up-to-date, and organized medical records.
- Serve as a resource and supervisor for Home Health Aides/CNAs, ensuring the plan of care is being followed.
Qualifications:
- Must be a graduate of an accredited school of nursing.
- Possess a current and unencumbered Registered Nurse (RN) license in the state of Florida.
- A minimum of one year of experience in a clinical setting; home health or hospice experience is highly preferred.
- Valid driver's license and reliable transportation.
- Strong critical thinking and problem-solving skills with the ability to work independently.
- Excellent communication, interpersonal, and organizational skills.
- CPR and First Aid certification.
- Ability to pass a background check and drug screening.
- A genuine desire to provide one-on-one, patient-centered care.
