Responsibilities
- Conduct comprehensive assessments to evaluate patients' physical, cognitive, and emotional needs related to daily activities and functional abilities
- Develop personalized care plans targeting areas such as assisting with activities of daily living (ADLs), mobility, and sensory processing
- Implement therapy interventions tailored to various populations including pediatrics, geriatrics, individuals with autism, disabilities, or post-acute conditions
- Assist patients with discharge planning by coordinating care transitions and providing education for continued progress at home or in community settings
- Collaborate with multidisciplinary teams including speech therapists, physicians, case managers, and educators to optimize patient outcomes
- Document patient progress meticulously through detailed reports and maintain accurate medical records in compliance with healthcare standards
- Engage in case management activities to monitor patient recovery trajectories and adjust treatment plans accordingly
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to occupational therapy practice
- Experience working with diverse populations such as pediatrics (including early intervention and school settings), geriatrics, and individuals with disabilities or autism spectrum disorder
- Proficiency in developing and managing care plans for inpatient facilities, skilled nursing facilities, outpatient clinics, home health agencies, and outpatient therapy centers
- Skilled in assisting patients with ADLs and mobility tasks to promote independence
- Ability to perform discharge planning effectively by coordinating resources for seamless patient transitions
- Familiarity with acute care environments such as ICU settings and inpatient units is highly desirable
- Excellent communication skills for collaborating with healthcare teams and educating patients on therapy exercises and safety measures
