About the Role
The home health RN performs home visits for a variety of hospice patients in the vicinity of Auburn, MA. This is a 13-week contract role with the opportunity to extend.

Responsibilities
- Conduct evaluations, re-evaluations, treatments, and discharge visits in a home health setting
- Develop individualized care plans to support patient recovery
- Complete timely and accurate documentation
- Educate patients and caregivers on home exercise programs and safety
- Communicate effectively with physicians, case managers, and agency partners
- Travel to patient homes within your selected coverage area
Requirements
- Active RN license for the state of MA
- Minimum 1 year of hospice experience within the past 3 years
- HCHB experience
- BLS/CPR certification
- Valid driver’s license and reliable transportation
Strong ability to work independently and manage caseloads
