DEPARTMENT: Early Head Start
JOB TITLE: Home Visitor
FLSA CLASS: Non-Exempt/ Full–Time
PAY RATE: $17.00/Hour
REPORTS TO: EHS Education Coordinator
JOB PURPOSE: Key responsibilities include: conducting a 1.5-hour home visit per family each week, maintain weekly lesson plans, screening and assessing children documenting and maintaining factual case management information on families assigned to the home visitor caseload, and entering that information into the database regularly as scheduled. Actively participating and implementing activities for family socializations. Working with Parents as the child’s first teacher. Active participation in the recruitment of children and families for enrollment in the program and follow-up on referrals as assigned.
EDUCATION AND WORK EXPERIENCE: High School Diploma/GED, CDA or Bachelor Degree with a minimum of one (1) years of case management experience with families, infants and toddlers who may be economically, physically, and/or emotionally at-risk. Will need to attain the Family and Community Development Certification.
KNOWLEDGE AND SKILLS: Working knowledge of community based social, mental health and childcare services. Effective written and oral communication skills. Planning and organization skills. Ability to provide training and technical assistance to parents and staff. Must be computer literate.
DUTIES AND RESPONSIBILITIES:
- Weekly home visits of 12-13 Early Head Start families.
- Prepare and maintain client files.
- Work with Caregivers as the child’s 1st teacher
- Weekly Lesson Plans
- Screening/Assessment of children
- Plan parent/child socializations with the assistance of caregivers
- Responsible for the monthly reporting, collecting and submission of in-kind and non-federal share contributions.
- Provide on-going education guidance and/or counseling as appropriate.
- Facilitate referrals to other PCCEO departments, community agencies, and/or specific treatment facilities in response to the families’ needs, such as housing, or disability services.
- Attend and maintain meeting notes from outside agency meetings as necessary for case management:
- Record information on assigned families, data entry through GoEngage and case management with adequate provisions for maintaining confidentiality.
- Home Visitor must observe all local, state and federal rules, which govern the Early Head Start program.
- Encourage and involve parents as volunteers in Early Head Start activities.
- The FCD Specialist will perform other duties assigned by the Director, Early Head Start program, Family Partnership Coordinator or President / CEO.
- Perform other duties as assigned.
- Must be able to perform the essential duties of the job description.
- Must have and maintain a valid Illinois Driver’s license, reliable transportation and insurance coverage as required by the State of Illinois.
