Essential Duties And Responsibilities
- Complete all required IBCCP forms for screening and diagnostic breast and cervical services, including retrieving clinical results into EHR when indicated.
- Consult regularly with Reproductive Health Coordinators on patient outreach and education; support clients directly when escalation is needed.
- Provide care management as indicated for all patients with abnormal reproductive health screens or positive diagnoses: provide outreach & education, clinical coordination & referrals, and report outcomes to oversight agencies. Delegate to RHCs only when appropriate.
- Follow-up with clients through clinic visits, phone calls and mail, as appropriate.
- Complete all forms and make contacts to enroll eligible IBCCP patients with cancer diagnoses or precancerous conditions into the Illinois Medicaid Program.
- Connect patients to care appropriate to their payer and clinical status.
- Participate in data entry and assist in quality assurance of information that is entered into Cornerstone system.
- Communicate with each client’s medical provider to obtain results of screening and diagnostic tests, as indicated by her medical status.
- Provide detailed information about IBCCP caseload to IBCCP Program Coordinator and Quality Assurance nurse.
- Attend all mandatory meetings and conference calls as required by IDPH and IBCCP.
- Attend regular departmental staff meetings and in-service training.
- Other duties as assigned.
Experience/Training:
- Graduate of an accredited school of nursing.
- 2 – 3 years clinical experience preferably in women’s health nursing, care/case management and/or community/public health.
- 2 - 3 years EMR experience preferred.
- Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.
- Strong interpersonal, verbal/written communication and organizational skills.
- Ability to work collaboratively and strong commitment to accountability.
- Current and valid State of Illinois nurse’s license.
- Current and valid CPR card.
- Must be able to remain in a stationary position 50% of the time.
- Must be able to move around the clinic site 50% of the time.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Ability to move as needed to perform necessary nursing skills such as wound care, injections, and other procedures. Must be able to transport from one site to another.
- Must be able to cover other shifts as necessary.
- Bilingual in English/Spanish required.
