What are the responsibilities and job description for the Perinatal Care Coordinator position at PCC Community Wellness Center?
Job Summary: The Perinatal Care Coordinator is responsible for coordinating care for perinatal patients primarily at home site, including outreach and scheduling of perinatal care, meeting with patients in-clinic 1-3 times during perinatal period, and supporting all health-related social needs identified. Completes in-scope tasks within their assigned PCH (Perinatal Child Health) site buckets in collaboration with Perinatal Nurse Care Manager.
Essential Duties and Responsibilities:
· The Perinatal CC will address barriers to care, increase patient motivation, and establish professional, dynamic relationships with patients, providers, referral sources, and other involved parties.
· Manage a case load of all pregnant and postpartum patients receiving care at home site(s), in collaboration with other assigned Perinatal CCs. Plan to meet all eligible patients in-clinic 1-3 times during pregnancy & postpartum period and enroll in Better Birth Outcomes program if applicable.
· Assist patients in scheduling and completing perinatal appointments at PCC and associated referrals as needed by coordinating between the patient, provider(s), and the referral source
· Advocate on patient’s behalf if needed to ensure completion of referrals; Contact referral sources when reports/results are outstanding, request results/reports not automatically received by PCC
· Conduct outreach for missed appointments, due, and overdue perinatal care per the high-risk patient protocol
· Decrease barriers to care, increase motivation, and foster open communication. Including assisting patients with scheduling transportation for medical needs, scheduling specialty or imaging care as directed by PCP or Perinatal Care Manager, and identifying resources to address patients’ health-related social needs; includes educating patients on completing these tasks directly.
· When requested by Perinatal Nurse Care Manager, with support of manager, Perinatal CC may join nurse on home visits to perinatal patients.
· Contribute to patient education materials and strategies to support care coordination
· Develop supportive services and tools to address common barriers to care for PCC patients; integrate these with other initiatives for health promotion/education and access to care
· Provides excellent customer service to internal and external customers
· Regularly attend and participate in monthly site team meetings
· Engages patients as active participants in their care
· According to manager discretion, supports various program areas, including but not limited to:
o Doula for PCC patients; these CCs work closely in conjunction with Director of Perinatal Education and their manager to ensure appropriate scheduling and completion of all duties.
o Prepare data and documentation and have charts prepped for all applicable Case Management sessions; completes all duties delegated by site Perinatal Care Manager related to case
management
o Lead Exposure Follow-up care coordination
o Early intervention follow-up care coordination
o Developmental wellness clinic support
o Referral prior authorization
o Collaborates with site Perinatal Care Manager to address abnormal newborn screens and outstanding newborn hearing screens
· Other duties as assigned.
Qualifications:
Knowledge of:
· Knowledge or experience with Microsoft Office
· Knowledge or experience with electronic health record software
Ability to:

Experience/Training:
· High School Diploma or GED equivalent,
· Associates or Bachelor’s degree preferred
· Constantly communicates with patients, families, and other healthcare providers. Must be able to exchange accurate information in these situations.

· Detail oriented with the ability to work with minimal/no supervision.
· Willingness to be part of a team-unit and cooperate in the accomplishment of departmental goals and objectives.
