We are seeking a passionate individual to lead our Saratoga Pediatric Subacute facility. The Children’s Healthcare Organization of Northern California (CHONC) is seeking a Director of Nursing (DON)
- CHONC – Saratoga Pediatric Subacute is in Saratoga, CA with 37 SNF beds

JOB PURPOSE AND SUMMARY
The DON will direct the nursing staff to provide the highest level of care while ensuring regulatory compliance. The DON will collaborate with the IDT members to coordinate the seamless care of the patient from admission to discharge. The DON can be assured they will work for a company that truly values the community it serves. The company is committed to making each day the best that it can be for every resident. This position requires a strong, seasoned DON with extensive long-term care management experience and the leadership skills required to ensure the best care possible for all residents.
PRINCIPLE DUTIES AND RESPONSIBILITIES
- Maintaining regulatory compliance & ensuring high quality medical care for all residents
- Proactively responding to survey and/or standards of care issues by complying with Title 22 and OBRA regulations
- Coaching and mentoring the nursing team and leading by example
- Developing / updating Nursing Department policies & procedures
- Developing, implementing, and evaluating programs to enrich the recruitment, retention and continuing education of the nursing staff
- Ensure nursing documentation is accurate and timely by periodic review
- Closely monitoring Nursing Department budget and facilitating resource stewardship
- Maintaining optimal employee relations and being a positive change agent
- Ensuring nurse and RT staffing hours are met for compliance with PPD
- Collaborate with Medical Director and other community physicians in providing excellent nursing care for the residents
- Review 24-hour report for change of condition, physician's orders and ensures appropriate care is delivered by nursing team to all the residents in the facility
- The DON is responsible for all the patients in the acute beds under the GACH license and d/p SNF
- Effectively determines the competency of the nursing staff along with the DSD
- Follow through with the Incident Reports and complete QA process as needed
- Must possess exceptional communication and customer service skills
- Must be able to effectively communicate in English – verbal and written
- Responsible for reviewing and approving admissions from referral agencies along with Medical Director and Administrator
- Collaborates with Medical Director and Discharge Planner to ensure safe discharge to next level of care
Qualifications:
Director of Nursing Qualifications
- Registered Nurse with 3 years of solid management experience in LTC
- License issued by the California State Board of Registered Nursing (BRN)
- Stable employment history
- Extensive knowledge of state regulations with regard to long term care
- Expertise in sub-acute care strongly preferred, pediatric experience preferred.
- Bachelors Nursing Degree (BSN)
- BLS and PALS from (AHA) American Heart Association.
PHYSICAL AND SENSORY REQUIREMENTS
(With or without the aid of Mech. Devices)
- Must be able to move intermittently throughout the workday.
- Must be able to cope with mental and emotional stress on the job.
- Must be in good general health and demonstrate emotional stability.
- Subject to frequent interruptions.
- Must be able to lift, push, pull and move a minimum of 50 lbs.
- Be able to work with patients, families, visitors, personnel, and government agencies under all circumstances.
- Must be able to assist in the evacuation of patients during emergency situations.
- May be requested to work overtime.
- Subject to potential falls, odors, burns from equipment, and hazardous materials. Also, to potential exposure to infectious waste, diseases, and conditions including TB, AIDS, and Hepatitis viruses.
WORKING ENVIRONMENT
- While performing the duties of this job, the employee occasionally works near moving mechanical parts and is occasionally exposed to toxic or caustic chemicals, risk of radiation and vibration.
- The noise level in the work environment is usually moderate.
- The employee must be able to work near other co-employees and concentrate efforts on tasks despite noise, interruptions, and general commotion.
Salary : $130,000 - $160,000
