What are the responsibilities and job description for the Licensed Vocational Nurse (LVN) - HOME HEALTH PRIVATE DUTY position at PAX HOME HEALTH?

Pax Home Health is hiring for a Licensed Vocational Nurse to work with Pediatric and Geriatric patients one-on-one in the home or school setting. Previous experience with feeding tubes, Seizure Management, tracheostomy, and ventilator management is preferred but not required. Paid training is provided if needed.
Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- On each assignment, provide skilled nursing care/services in accordance with prescribed orders.
- Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
- Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
Qualifications:
- Currently licensed as an LVN in the state in which the LVN will practice.
- Current TB or Chest X-Ray.
- Current BLS card.
- One year of prior professional nursing experience preferred
