In search of a QIDP for 2 - 16 bed group homes for intellectually disabled adults. Hours can be flexible, but most work must be done in one of the homes. Must spend time in both homes in our Northern area (Pekin and Canton) and be able to travel to corporate office monthly and other trainings throughout IL as needed.

Must have a Bachelors Degree with Social Service credits to qualify as a QIDP in the State of Illinois

Duties Include:

Duties can be delegated to others, but it is your responsibility to ensure that they are completed accurately.
When it comes to the individuals care and programs, some of these are more guidelines than actual policy. It is always expected that each person’s care is individualized to what best fit THEIR specific needs.

Immediately

Behavior Incident Reports

Review, sign and implement new programming if needed

Each Shift

· Read log- Deal with any issues from log as soon as possible

· Assist with Supervision of Staff- Disciplinary Action if needed (only if individuals programming needs are not met)

· Make time for the residents- Do best to make time daily to talk with residents. IF necessary, schedule times or set limits for this to help with attention seeking issues

· Supervise/Check supper- Ensure monitoring of resident and programs are being implemented (as possible, since most QIDPs travel)

· Med changes (psych meds)

Ensure intervention programming is changed to reflect medication change and
send for guardian signature

Monthly

· Monthly Q progress notes

Q notes written in social notes section of book

§ Notes should just include general observations from the month and address any major issues that have arisen and how they were handled

·Attend Monthly Staff Meeting and In-services

o Try and fit staff meetings into schedule at each house

o Discuss all new items/issues for that month which involves resident care

· Workshop Visit

o Try to complete visits at different times different months to see variety of activities

o Make sure any issues from visit is discussed with the Care Coordinators as soon as possible

o QIDP must attend visits at least quarterly per house

· Monthly Summaries

o Need completed and signed by the end of the following month or sooner when possible. This allows to gather all info needed from workshops, directors, RSD, etc.

o Distribute summaries as needed, exp workshop, ISC, etc.

· Program Updates- May delegate if needed

o UD and AD should give you lists of program updates needed by the 1st week of the month

o These will include programs med and programs that are expired

§ Expired-goal objective need revised to encourage ability to meet goal

§ Met-decrease number of verbal prompts. If independent, increase days, change to maintenance goal, or revise to next step to work towards long term goal/independence

§ Distribute as needed. Ensure copies give to proper staff to implement and workshop receives any updated goals they are working towards

Quarterly

· Assist RSD as needed

· Attend Quarterly Human Rights/Behavior Management Meetings and other quarterly meetings as needed (if possible)

Yearly

· Review/complete annual IDT’s. Ensure maximum growth and programs identified match assessments and needs identified

· Complete Annual IDT Assessments

· Write and implement all new programming and interventions discussed at IDT meeting

As Needed

· Screening of Potential Residents

· Admission of New Residents

o Assist/Review 30 day IDT

o Write and implement all new programming and interventions discussed at IDT meeting

· Discharge of Residents

· DSP and QIDP Training as needed

· Complete all addendums to ISP/PCP as warranted. Ensure all programming identified with addendum is written and put into place.

These duties are a brief overview of responsibilities. QIDP will be subject to complete tasks as requested by Administrator or Ownership.

Job Type: Full-time

Pay: $51,500.00 - $53,000.00 per year
