About the LPN Position
As an LPN at Patriot Village, you will play an important role in providing quality nursing care and supporting the health, safety and well-being of our residents.
You will work alongside our nursing leadership and care team to provide compassionate, individualized care while following established policies, procedures and resident care plans.
Responsibilities include:
- Providing nursing care in accordance with resident care policies, procedures and care plans
- Administering medications and treatments as ordered by the physician
- Monitoring residents for responses to medications and treatments
- Completing accurate and timely nursing documentation
- Performing resident assessments and identifying changes in condition
- Providing care for residents with a variety of needs, including dementia, neurological conditions, dialysis, IV therapy, infectious disease and wound care, as applicable
- Providing catheterization and tube feeding care as needed
- Maintaining resident safety and well-being
- Following all applicable state and federal regulations
- Maintaining HIPAA compliance and resident confidentiality
- Working collaboratively with nursing staff, residents and families
- Performing additional duties as assigned
Qualifications
- Current and valid LPN license
- Basic Life Support (BLS) certification
- Experience in long-term care, skilled nursing or assisted living preferred
- Strong communication and organizational skills
- Ability to provide compassionate care to adult and geriatric residents
- Ability to work effectively as part of a team
- Familiarity with electronic health records (EHR) systems
- Experience with catheterization, tube feeding, medication administration and other nursing treatments preferred
