We recognize family as the cornerstone of our company, and want you to be part of a team that values your contributions and well-being.We value our families and community:Flexible SchedulePaid time offMinimal holiday rotationHardship Program- program to support staff during challenging timesPatriot Cares- nonprofit for community outreachHow we support you professionally:This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rateMileage reimbursement for any company travelMedical/Dental/Vision Insurance/Supplemental insurance (begins immediately upon hire)Company 401K with 4% company matchingThe responsibilities of our Physical Therapists include:Provide initial and ongoing comprehensive assessments of the patient's needs, including Outcome and Assessment Information Set (OASIS) assessmentsEstablish a treatment plan in consultation with the physician and revises it with the approval of the physician as necessaryAssist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical TherapyAssist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability testsTeach, supervise, and counsel the family and patient in the physical therapy programWrite clinical/progress notes on the date of the visit and incorporates same in the patient's record within 72 hours.Job Conditions:The ability to drive and ability to access patients' homes which may not be routinely wheelchair accessible are requiredHearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient careOn occasion, may be required to bend, stoop, reach and move patient weight up to 250 pounds; lift and/or carry up to 30 poundsQualifications:Must be a graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy AssociationMust be licensed or registered by the StateMust have at least one year of institutional/home health experienceMust have criminal background checkMust have a current CPR (BLS) certificationPatriot Homecare is an Equal Opportunity Employer.
