The responsibilities of our Wound Care Nurses include:
- Perform comprehensive wound assessments, including measurement, staging, and documentation of wound characteristics
- Develop and implement individualized wound care plans in collaboration with the physician and interdisciplinary team
- Administer prescribed treatments and procedures, including wound debridement (as applicable by license and certification), dressing changes, and infection management
- Educate patients and caregivers on wound care management, nutrition, skin care, and prevention strategies
- Monitor and document wound progression, response to treatment, and any signs of complications
- Communicate effectively with the physician and care team to ensure continuity of care and timely updates on patient condition
- Maintain accurate and timely clinical documentation in accordance with agency and regulatory requirements
- Participate in quality assurance initiatives and wound care performance improvement activities
- Ensure compliance with infection control standards and universal precautions at all times
- Serve as a clinical resource for staff nurses regarding wound management best practices
- All other duties as assigned
Qualifications:
- Graduate from an accredited School of Nursing
- Licensed in the state as an RN or LPN
- Certification in Wound Care (WCC, CWON, CWCN, or equivalent)
- Minimum of 1-2 years of clinical nursing experience; home health or wound care experience preferred
- Strong knowledge of wound healing principles, wound dressings, and negative pressure therapy (VAC)
- Knowledge of Medicare and Medicaid guidelines
- Criminal background check
- Must have a current CPR certification
Job Conditions:
- Valid driver's license required
- Ability to access patients' homes, which may not be wheelchair accessible.
- Sufficient hearing, eyesight, and physical dexterity for patient assessments and care
- Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
