We recognize family as the cornerstone of our company, and want you to be part of a team that values your contributions and well-being.We value our families and community:Flexible ScheduleHardship Program - program to support staff during challenging timesPatriot Cares - nonprofit for community outreachHow we support you professionally:This position is paid at a competitive per diem visit rateThe responsibilities of our Home Health Aides include:Follows the plan of care to provide safe, competent care to the clientHelps maintain the patient to maintain good personal hygiene and assists in maintaining a healthful, safe environmentAssists the patient with ambulation as ordered by the physician and approved and supervised by the nurseAssists the therapy personnel as needed with rehabilitative processesEncourages the patient to become as independent as possible according to the nursing care planReports any change in the patient's mental or physical condition or in the home situation to the staff nurseConforms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinatedAll other duties as assignedQualifications:Preferably a high school diploma or equivalentMust have valid STNA LicenseMust have completed a minimum of 75 hours of training which includes an introduction to personal care servicesMust be free from health problems that may be injurious to patient, self and co-workers and must present appropriate evidence to substantiate thisMust have current CPR (BLS) licenseMust have criminal background checkJob Conditions:Valid driver's license requiredAbility to access patients' homes, which may not be wheelchair accessible.Sufficient hearing, eyesight, and physical dexterity for patient assessments and careStandard aide activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patientsPatriot at Home is an equal opportunity employerJob Type: PRNWork Location: On the road
