Team members will provide direct care and support in clients’ homes, including assistance with activities of daily living, basic medical tasks as allowed by licensure, monitoring client conditions, and reporting changes to supervisors. Responsibilities may include personal care, mobility support, meal preparation, light housekeeping, medication reminders, and companionship to enhance clients’ quality of life. RNs and LPNs may perform nursing assessments, care planning, wound care, and coordination with physicians, while CNAs, HHAs, Homemakers, and Companions focus on hands-on daily support. All staff are expected to follow care plans, maintain accurate documentation, uphold safety and infection control standards, and communicate respectfully with clients, families, and the care team.

Qualifications
