What are the responsibilities and job description for the Nurse Practitioner position at Patients Choice Primary Care?
This position is 2 days a week in Pahrump, Nevada.
- Conducts medical evaluations of predominantly elderly, frail, chronically ill homebound patients in their home environment.
- Documents visit via EMR *locked in 72hours.
- Develops a treatment plan in collaboration with the patient and family and their goals of care.
- Evaluates patient medication and writes/orders prescriptions as needed.
- Orders and interprets diagnostic tests relevant to patient’s needs.
- Collaborates with multidisciplinary team members.
- Facilitates the patient’s transition within and between health care settings.
- Educates and counsels patients and their families regarding health care options.
- Makes appropriate referrals to home health or hospice as needed.
- Facilitates access to appropriate supportive care services.
- Serves and protects the health care provider practice by adhering to professional standards, policies and procedures, federal, state, and local requirements.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
- Seeing on average 6-10 patients per day in homes and in facilities.
- Reliable Transportation
- Masters/Doctorate in Nurse Practitioner
- State License as Nurse Practitioner
- DEA License
- CPR
