We are looking to bring on a Social Worker (BSW or LSW) as a Care Coordinator that will work with the Patient Sortal Health medical team to coordinate care for Patient Sortal members. MUST have care coordinator/case management experience in New Jersey.

The individual in this position will function as part of an interdisciplinary team, supporting Patient Sortal Health and is responsible for managing Patient Sortal members through their clinical care coordination program. This individual will work along with an interdisciplinary team to connect participants with community and social services, ultimately with the aim of improving patient care outcomes and quality of life and reducing unnecessary healthcare utilization. This is a virtual position for the most part with room for face to face capacities when necessary or desired throughout New Jersey.
Position Overview:
We are seeking a compassionate and detail-oriented Medical Case Manager / Public Benefits Navigator to join our team in New Jersey. This role is ideal for someone with strong experience in public benefits enrollment, insurance navigation, and medical case management, particularly in complex and high-need situations. The right candidate will be comfortable working directly with patients, navigating county assistance offices, and helping individuals access critical services including Medicaid, SNAP (food stamps), and other local/state programs.
Key Responsibilities:
- Conduct in-depth assessments of patients eligibility for public benefits and medical resources
- Assist patients with enrollment in Medicaid, SNAP, and other public assistance programs
- Troubleshoot and resolve complex enrollment challenges
- Communicate with county assistance offices, insurance providers, and healthcare systems on behalf of patients
- Coordinate services and maintain up-to-date documentation and case notes in EHR system
- Advocate for patients facing barriers to care or benefit access
- Connect to outside providers such as behavioral health specialists and drug and alcohol support programs. Connect with pharmacies and labs as needed.
- Navigate local community resources such as food banks, clothing, shelter systems, government phone programs.
- Communicate daily with an interdisciplinary team including medical providers.
- Address crisis situations when needed
Qualifications:
- Minimum of 2-3 years’ experience in medical case management, social work, public health, or a related field
- Deep understanding of New Jersey’s public benefits system, including Medicaid, SNAP, and local county services
- Experience working with diverse populations, including individuals with complex medical and/or social needs
- Strong interpersonal and advocacy skills
- Ability to manage and maintain accurate records
- Ability to be creative when assisting patients
- A passion for helping those affected by the justice system
Job Type: Full-time
Pay: $52,000.00 - $60,000.00 per year
