Patient Centered Services and Seniors Home Healthcare are seeking an experienced PHC / PAS Program Coordinator to oversee and coordinate services for our Primary Home Care (PHC) and Personal Assistance Services (PAS) programs. The ideal candidate must have prior experience working with Texas Medicaid PHC or PAS programs and hands-on experience using HHAeXchange.
Responsibilities:
- Coordinate PHC / PAS services for assigned patients
- Schedule attendants based on authorized hours
- Manage caregiver assignments and coverage
- Communicate with patients, attendants, and families
- Monitor utilization of authorized hours
- Ensure compliance with HHSC and agency policies
- Maintain accurate documentation and tracking logs
- Handle call-offs and arrange replacement staff
- Use HHAeXchange for scheduling, authorizations, and EVV tracking
- Support administrative and operational tasks
Requirements (MANDATORY):
- Previous experience with PHC or PAS program
- Experience working in a home care or home health agency
- Hands-on experience with HHAeXchange
- Knowledge of HHSC requirements
- Experience scheduling caregivers/attendants
- Strong organizational and multitasking skills
Preferred:
- Bilingual in Spanish is a plus
- Texas Medicaid program experience
- EVV management experience
- Coordinator or scheduler background
- Ability to work in fast-paced environment
Schedule:
Monday – Friday
Office hours 9:00 am - 5:00 pm
Pay: $20.00 - $25.00 per hour
Work Location: In person
