Pathways Transition Programs (PTP)
Founded in 1991, Pathways Transition Programs (PTP) is a private mental health provider dedicated to serving children and their families. With six office locations across north-central Georgia, PTP provides services to more than 20 counties. Our team of over 100 full-time professionals includes a licensed psychiatrist, psychologists, counselors, and social workers, supported by highly trained and supervised behavioral aides. Together, we are committed to protecting children and strengthening families.
Community-Based Clinician Role
- Provide trauma-informed individual and family therapy in home, school, and/or office settings in Barrow County, Georgia
- Maintain a full-time caseload (25–30 face-to-face counseling hours weekly)
- Collaborate with children, families, schools, and community partners to build skills and achieve treatment goals
- Deliver clinical services through all phases of treatment—Engagement, Empowerment, and Transition—following PTP protocols
- Document services in accordance with PTP and payor requirements using CareLogic, our HIPAA-compliant electronic medical record system
- Participate in supervision, clinical team meetings, and regular agency trainings
