Intake Coordinator


EXEMPT/NON-EXEMPT: Non Exempt

SUPERVISOR’S TITLE: INTAKE DIRECTOR 

WORK SCHEDULE: Monday through Friday, 8:30a.m. To 5:00 p.m., Or later depending on job related duties. Occasional weekends and holidays

SUMMARY OF POSITION RESPONSIBILITIES:

The intake coordinator will meet the criteria of a Qualified Professional, who will assist with scheduling and coordinating and completion of all intakes in relation to all services offered by the site.  The intake coordinator will collaborate with all providers involved in the intake process (including physicians) and maintain an active schedule to ensure a smooth transition of services from provider to consumers.   


ESSENTIAL DUTIES: 

  1. Complete all entrance documentation, providing explanation of services and forms to each client and guardian prior to assessment.

  2. Complete all insurance verification and checks eligibility, 

  3. Applies for IPRS funds for clients without insurance, ensuring all documentation is kept and complete in a timely manner.

  4. Check enrollment with MCO and/or create enrollment if needed.  

  5. Verifies insurance copays with BCBS and other sources as requested. Distribute information to appropriate staff.

  6. Assist with completion of 30 day audits, audits should be complete for all clients by the 5th of the following month for the assigned site.

  7. Access to Care maintenance.  Add all referrals/ intakes to the ATC spreadsheet and assure each column is updated and accurate (daily).

  8. Maintain authorization listing in the EMR, adding units and uploading all official authorization letters.

  9. Uploads client documentation into the medical record (EMR) related to the authorization.

  10. Maintain safety and confidentiality within the intake process

  11. Maintain appropriate professional conduct while working with individuals and families and interacting with staff.

  12. Distribute appropriate and accurate information to professional staff, physicians and relevant clinical providers related to consumers.

  13. Maintain a positive and professional relationship with all stakeholders and community networks.

  14. Communicates information according to company policy and procedures as well as the service definition. 

  15. Demonstrates sensitivity to age, developmental and cultural needs of clients.  

  16. Attend stakeholder and community meetings as assigned by the supervisor.

  17. Provide coverage for administrative assistants daily for 1 hour during lunch.

  18. Performs miscellaneous job-related duties as assigned by supervisors, to include but not limited to performing Administrative Assistant duties as needed and requested.

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS: 

  1.   Intake Coordinator will have a minimum of a bachelor’s degree in a related health field with a strong working knowledge of the company policies, mission, and overall objective.  

  2.   Must have a bachelor’s degree in a Human Service field with two years of full time post bachelor experience or a Bachelor's degree in a non-Human Service field with four years of full time experience with the population served.

  3.   Intake Coordinator will have all required company training met within a 30 day period of hire.  Also include 20 hours service definition specific training within 90 days of hire.  (See company training requirement outline for more details)

  4.   Have no substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry.

  5.   All facilities or services shall require that all applicants for employment disclose any criminal conviction. The impact of this information on a decision regarding employment shall be based upon the offense in relationship to the job for which the applicant is applying.

  6.   Ability to apply the concepts of customer service in the workplace. . Ability to use good professional judgment. Detail oriented. Ability to work independently, with minimal direct supervision.

  7.   Excellent organizational, prioritization, and interpersonal communication skills. Excellent oral and written communication skills

  8.   Ability to handle multiple tasks and meet critical deadlines.  Ability to meet short and long-term goals and project deadlines.

  9.   Respect for the confidentiality of all communications required within job duties, including client/employee records and documents, and electronic transmission by voice, data, and wire.

  10.  Experience with multi-cultural/multi-lingual populations and multi-disciplinary teams.

PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drive a motor vehicle.





WORKING CONDITIONS:

Work is performed in an office or other environmentally controlled room; Work may expose Intake Coordinator to contagious or infectious diseases; Work may expose Intake Coordinator to dangerous and volatile situations; which could result in bodily injury.
