Title: Care Coordinator (Night Shift)
Classification: Non-Exempt
Reports to: Program Manager
Role Summary
The Care Coordinator position will provide a variety of support to the Pathways to Housing organization and patients that are served in the SCHC HOPE Medical Respite program. This position includes housekeeping work and supporting patients who need respite/recuperative care and are experiencing homelessness. The Respite program is operated in a group facility setting with 24 hour awake staff. This position works with a diverse and vulnerable population.
Responsibilities/ Essential Functions

1. Develop rapport, communicate effectively and build relationships with patients who are placed in respite program.
2. Daily cleaning, stocking and turning over the facility for the next day.
3. Conduct and maintain daily check-ins. Monitor patient daily activities and document interactions, along with patient needs in documentation system.
4. Assist with patient intakes as needed.
5. Inform Program Manager of any patient’s medical or hospitality issues in a timely manner.
6. Mitigate any patient issues that arise per protocol.
7. Perform basic housekeeping duties including laundry.
8. Conduct perimeter and client checks.
9. Comply with all policies, procedures and protocols including safety protocols.
10. Assist with maintaining daily house schedule.
11. Assist patients with rule compliance.
12. Assist patients with laundry and hospitality needs as necessary.
13. Conduct turn overs in client sleeping area as needed.
14. Provide daily report to management via email.
15. All shifts require staff to stay awake and alert throughout the entire shift.
16. Other duties as assigned.

Qualifications
1. Possess a valid CPR and First Aid certificate once hired.

Work Environment
This job typically operates in a variety of conditions at the employer’s location. The employee must be able to work in a variety of weather conditions and temperatures outside, as well as within the building. The noise level may be moderate at times.
Physical Demands
This is a moderately physical position that will require bending, stooping, stretching, twisting, reaching, pulling and regularly lifting up to 40 pounds and occasionally up to 50 pounds, use of hands to grasp, move or assemble objects, wrist bending, use of equipment and materials and dealing with the outside elements. The position requires good manual dexterity (hand, hand with arm, two hands) and multi-limb coordination. This position will also require sitting, movements required for use of a computer including typing, speaking to others, and hearing.
Position Type and Expected Hours of Work
This is a Full-time position, up to 40 hours. Days and hours of work include weekends and holidays. Overtime hours may be required as needed. Shift hours: Sunday-Wednesday Midnight-10:00am
Travel
Any required travel is primarily local. Company provides the required transportation vehicle. Travel expenses will be reimbursed at the current standard mileage rate reimbursement.
