Pathways Residential Care is a Michigan Adult Foster Care provider specializing in residential placements for high-behavior adults. We believe everyone deserves dignity, person-centered care, and a real home. We're growing, and we're looking for someone who shares that heart to help us reach more people and run a tight, trustworthy operation.
Requirements (applications that don't meet these will not be considered)
- Bachelor's degree in Social Work (BSW)
- At least 2 years of experience in Michigan Community Mental Health (CMH), case management, intake/access, or AFC/residential behavioral health
- Working knowledge of Michigan CMH referral processes, person-centered planning, and LARA Adult Foster Care rules
- Able to pass a criminal background check through the Michigan Workforce Background Check system
- Valid Michigan driver's license and reliable transportation (this role includes regular travel to CMH offices and partners)

What you'll do — Referrals
- Build and nurture genuine relationships with CMH case managers and referral partners
- Be the warm, trusted face of Pathways out in the community
- Keep our homes full by staying on top of referral opportunities
- Represent us with care and professionalism everywhere you go
What you'll do — Compliance
- Run our monthly self-inspection checklist and keep our homes always inspection-ready
- Keep documentation and staff records accurate and audit-ready for LARA
- Help us prepare for LARA inspections and stay ahead of every requirement
- Partner closely with our administrator to keep everything running smoothly
Pay
$24.11–$29.04 per hour, based on experience
