JOB TITLE: Case Manager – Full Time
REPORTS TO: Clinical Director
EMPLOYMENT TYPE: Full-Time
SALARY RANGE: $52,000 – $62,000 per year
SUMMARY:
The Case Manager is part of a compassionate clinical team supporting recovery through case management and social work services for adults impacted by chronic and severe mental illness. This role serves individuals living in supportive housing, in the community, and those attending the Pathways Club.
The ideal candidate is a flexible, independent, and collaborative team player with strong documentation skills and the ability to work effectively in an Electronic Health Record (EHR) system. Experience working with individuals with serious and persistent mental illness across residential and community settings is strongly preferred.
RESPONSIBILITIES:
Clinical & Case Management Services
- Assess client strengths, needs, and abilities to develop individualized, client-centered treatment plans in collaboration with clients, supervisor, and community supports.
- Conduct quarterly assessments covering residential, psychiatric/mental, social/vocational, financial, and community functioning.
- Provide individual counseling and make referrals for group or family counseling as needed.
- Provide crisis intervention and verbal de-escalation support when necessary.
Service Coordination
- Collaborate with community agencies and referral sources to coordinate client services.
- Assist clients with:
- ADL skills (money management, shopping, housekeeping, hygiene, leisure activities, travel training, etc.)
- Accessing and managing benefits (federal, state, local)
- Scheduling and attending medical/psychiatric appointments
- Mediation with landlords, family members, and others
- Ensure communication of medication changes to residential staff and maintain accurate medication documentation.
Documentation & Compliance
- Maintain timely and accurate documentation in the Electronic Health Record (EHR), including progress notes, treatment plans, and discharge summaries.
- Complete state-required documentation (DDAP, Mental Health Waiver, Time Studies, etc.).
- Complete CARF-related responsibilities, including chart reviews, trainings, and required forms.
Team & Program Participation
- Participate in clinical meetings, staff meetings, and community meetings related to client care.
- Investigate case situations and present relevant clinical information for treatment planning.
- Provide orientation to new clients and staff regarding program philosophy and expectations.
- Support transitions for clients moving between case management programs.
Professional Development
- Engage in ongoing professional development and training.
- Stay current with best practices in mental health, case management, and social work.
- Maintain MSW licensure or actively pursue licensure requirements, including supervision and exams.
- Complete special projects and additional duties as assigned.
Required Knowledge, Skills, and Abilities
- Knowledge of human development, psychopathology, and social work/case management principles
- Strong understanding of serious and persistent mental illness
- Excellent written and verbal communication skills
- Strong organizational and documentation skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of community resources and entitlement programs
- Proficiency in Microsoft Office 365 and ability to learn EHR and state systems
- Ethical practice aligned with the Social Work Code of Ethics
- Creative problem-solving skills and professional boundaries
QUALIFICATIONS:
- Bachelor’s degree in a mental health-related field required (MSW preferred)
- Minimum of 2 years’ experience working with individuals with mental illness
- Strong interpersonal, communication, and multitasking skills
- Valid driver’s license, clean driving record, and ability to use agency vehicle
