Role Description
This is a full-time, on-site role, located in Harrisburg, PA, for a Mental Health Case Manager. The Case Manager will provide case management services, assist individuals in accessing mental health resources, and develop personalized care plans. Responsibilities include assessing client needs, coordinating support services, maintaining documentation, and fostering strong professional relationships to promote client well-being. Compensation includes up to a $5,000 bonus.
Qualifications
- Proficiency in Social Work or Psychology with a focus on helping individuals improve their mental health and quality of life
- Experience in Case Management and delivering Case Management Services efficiently
- Strong Interpersonal Skills to build trust and maintain effective communication with clients and stakeholders
- Organizational and problem-solving abilities, with attention to detail
- Bachelor’s degree in Social Work, Psychology, or a related field is required
- Familiarity with mental health systems, community resources, and related policies
- Prior experience in a similar role or in community-based human services is a plus
- Valid driver’s license and reliable transportation
