Key Responsibilities:

  • Under the Supervision of the MH Intake/Administrative Supervisor:
  • Adheres to the Code of Ethics with consumers, co-workers, providers, and other stakeholders.
  • Interview prospective mental health consumers to obtain psychosocial data and other relevant information necessary to determine eligibility for services and specific service needs. Arrange for any professional evaluations which may be necessary to further ascertain consumer’s eligibility and service needs. Clarify and explain to consumers the services available through the agency and the County MH/MR Program.
  • Assesses emergency and “immediate need” situations and makes appropriate referrals. Facilitates voluntary and involuntary commitments and admissions to inpatient psychiatric facilities.
  • Ensures that consumers undergo an initial financial liability determination and annual liability while on administrative caseload.
  • Utilizes strengths/needs assessment and environmental matrix to determine level of case management services needed. Completes referrals and authorizations for appropriate levels of case management services. Makes initial referrals to provider agencies.
  • Arrange for any professional evaluations which may be necessary to further ascertain consumer’s eligibility and service needs for purpose of liabilities. Clarify and explain to consumers the services available through the agency and the County MH/ID Program. Complete referral to County Assistance Office for MA insurance if appropriate.
  • Follows established protocol and procedures for gathering consumer intake information.
  • Provide individuals pending TCM service with a bi-weekly phone contact.
  • For individual’s matrixed on an Administrative level of care:
  • Arranges, and when necessary, authorizes mental health services. Coordinates and facilitates consumer movement through the Service Delivery System to ensure continuity of care and the provision of services in a timely and organized manner.
  • Facilitate interagency team meetings and ISPT meetings.
  • Monitors consumer’s participation and progress in planned services by completing bi- monthly phone contact with individual.
  • Complete a community face to face level of care assessment every six months for individuals’ matrixed at the administrative level of care.
  • Complete Matrix, Releases of Information, and Liability yearly face to face with individuals in the community.
  • Develops and sustains an ongoing relationship with the consumer and significant others to ensure implementation of MH Services.
  • Maintains liaison and ongoing contact with service providers.
  • Participates in treatment and pre-discharge planning for consumers hospitalized in community psychiatric facilities and state institutions.
  • Advocate on behalf of consumers to service providers and others significant to the care and treatment of the consumer.
  • Responds to and assesses by phone emergency and “immediate need” situations and makes appropriate referrals.
  • Prepare necessary reports and correspondence.
  • Participate in staff meetings. Participates in orientation and ongoing training activities.
  • Apply agency mission, vision, and values to everyday work.
  • Apply resiliency and recovery philosophy to everyday work.
  • Maintains appropriate records and documentation of activities.
  • Develops and maintains relationships with supervisory staff, coworkers, mental health service providers, and community agencies.
  • Handles special projects and assignments delegated by the Intake/Administrative Supervisor.

Qualifications:

Minimum Experience and Training:

  • High school diploma with 12 semester credit hours in social sciences AND two years of experience in public or private human services, with one year of direct contact with mental health consumers.
  • OR a high school diploma AND five years of mental health direct care experience.
  • Act 33/34 clearances.
  • Access to a vehicle for work-related travel.
  • Prior case management experience preferred.

Preferred Experience and Training:

  • Bachelor’s Degree in sociology, social welfare, psychology, or related field.
