Lead Care Manager - Job Summary
Growing faith based, non-profit, private, foster family and enhanced family care management agency seeking a dedicated and compassionate Care Manager to join our team. The Care Manager will play a crucial role in coordinating member care and ensuring that individuals receive the necessary support throughout their health journey. This position requires strong communication and case management skills.
Responsibilities
- Provide whole person centered care management for a caseload of children or at risk families in the community.
- Conduct comprehensive assessments to identify needs and develop individualized care plans.
- Coordinate care among various healthcare and community providers to ensure continuity and quality of care for members.
- Monitor member progress and adjust care plans as necessary based on ongoing assessments.
- Maintain accurate documentation in compliance with HIPAA regulations and organizational policies.
- Collaborate with interdisciplinary teams within primary care, behavioral health, and settings.
- Advocate for members' needs within the healthcare and social systems to promote optimal outcomes.
- Stay informed about best practices in case management and participate in ongoing professional development.
Requirements
- Prefer a Bachelors degree in Social Work or nursing, other equivalent education and experience may be considered.
- Proven experience in case management or a similar role within a clinical or social services setting.
- Strong knowledge of HIPAA regulations and terminology.
- Excellent communication skills, both verbal and written, with the ability to build rapport with children and families.
- Ability to work collaboratively within a multidisciplinary team
- Proficient in using electronic health records (EHR) systems for documentation.
