Pathfinder is looking for a Connection Leader to manage daily services in Medicaid Waiver-funded programs and lead a team of Direct Support Professionals (DSPs). In this role, you’ll be a mentor, advocate, and champion for individuals with intellectual and developmental disabilities—helping them live active, empowered, and connected lives.
You’ll ensure high-quality support at our service locations, coordinate staffing, provide direct care as needed, and guide Person-Centered Planning efforts. As a Connection Leader at Pathfinder, You Will:
Lead Services and Support People
- Manage daily operations at assigned service locations.
- Act as a mentor, teacher, and advocate for individuals supported.
- Guide DSPs to deliver high-quality, person-centered care.
- Assist individuals in daily living, goal achievement, and community engagement.
- Ensure that Person-Centered Individualized Support Plans (PCISP) and safety measures are followed.
Supervise and Support Staff
- Train and mentor DSPs to maintain consistent, high-quality service.
- Oversee staff scheduling, timesheets, and performance management.
- Organize staff meetings and fill in during staffing shortages when needed.
Promote Quality and Advocacy
- Support individuals and families in building natural support systems.
- Advocate for individuals during quarterly and annual team meetings.
- Ensure funding and programming align with each individual's needs.
- Review and monitor waiver budgets to meet service and support goals.
Maintain Safety and Compliance
- Monitor environmental safety at service sites.
- Submit maintenance requests and oversee repairs.
- Ensure documentation, health records, and progress reports are completed accurately and on time.
- Participate in audits and support organizational compliance.

Experience & Qualifications
- High school diploma or GED required.
- 18 years or older.
- Prior experience supporting individuals with disabilities preferred.
- Valid driver’s license, insured vehicle, and a safe driving record.
- Ability to lift up to 50 pounds and work a flexible schedule as needed.

Requirements
Preferred Minimum Qualifications
- Minimum of a high school diploma/GED.
- At least 18 years of age.
- Basic language, written, and computer skills necessary for communication and documentation.
- Demonstrated ability to support people with disabilities.
- Ability to work a flexible schedule if needed.
- Crisis prevention, intervention, and teaching skills.
- Valid driver’s license, insured vehicle, and safe driving record.
- Ability to speak, hear, and read the English language.
- Lift up to 50 pounds, push and pull against potential resistance, and implement non-physical and approved physical interventions and CPR with people supported when needed. He/she shall have adequate vision, writing, and documentation skills to perform the essential functions of the job.
