GENERAL DESCRIPTION OF POSITION

Develops and maintains all DDTCS client files for accuracy of information and current information. Assists instructors and counselors with assigned paperwork. Other duties include but are not limited to: greeting customers, answering phone and taking messages, record keeping, filing, updating departmental employee files, employee payroll and maintaining inventory.

MONDAY-FRIDAY, 8:00AM-4:00PM

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Develops and maintains all DDTCS client files, including filing all necessary paperwork and obtaining required signatures.
  2. Completes the initial development, assessment, and updating of each trainee's Individual Program Plan.
  3. Initiates monthly data collection goal, data sheets, and distributes to instructors.
  4. Reviews monthly data collection sheets completed by instructors.
  5. Tracks all monthly and quarterly reports.
  6. Copies and distributes annual IPP information for other departments within Pathfinder.
  7. Checks progress notes and daily task area notes for accuracy.
  8. Schedules and conducts all annual IPP conferences with staff, client, family, and all other departments working with the individual.
  9. Assists in all workshop activities throughout the year.
  10. Responsible for completing and maintaining IPP. Completes the initial development, assessment, and updating of each trainee's Individual Program Plan and maintains and updates IPPs for each client yearly.
  11. Assists in driving the van to pick up or deliver trainees as necessary.
  12. Answer telephone, take messages or transfer calls to appropriate individual and greet customers.
  13. Order, purchase, invoice, stock and deliver office supplies to all staff and work areas in the department.
  14. Assist staff in maintaining departmental records.
  15. Responsible for all incoming and outgoing mail.
  16. Make use of databases, spreadsheets and other programs to perform daily tasks.
  17. Operate and maintain office equipment such as fax machines, copiers and multi-line phones and make use of programs such as MS Word, MS Excel, MS Outlook and other essential applications.
  18. Schedule and confirm appointments for employees and clients.
  19. Assist with payroll when necessary.
  20. Perform any other related duties as required or assigned.

ADDITIONAL DUTIES

Responsible for a caseload of 20 Waiver clients. Responsible for initiating and completing all necessary procedural steps for the establishment and implementation of direct provision of supported living services for individuals receiving Medicaid Waiver services. Responsible for coordinating and monitoring each individual's service plan, services delivered, and progress. Duties also include hiring, training, supervising, disciplining, and evaluating Supported Living staff; overseeing payroll; ensuring billing integrity by reviewing and approving MITC time, attendance, and timecards; reviewing and approving direct-care travel; and maintaining accurate documentation of work performed.

EDUCATION AND EXPERIENCE

High school or GED, plus specialized schooling and/or on the job education in a specific skill area; e.g. data processing, clerical/administrative, equipment operation, etc, plus 0 to 6 months related experience or training. Or equivalent combination of education and experience.

REQUIRED CERTIFICATES, LICENSES, REGISTRATIONS

Valid Driver's License, current auto liability insurance and registration and able to drive a Pathfinder vehicle.
