Passionate Heart Hospice is seeking a compassionate exceptional Hospice CHHA to join our care plan team in LA & Southern California. The ideal candidate will provide quality end-of-life care to patients in need. This position offers an opportunity to make a meaningful difference in the lives of individuals and their families during challenging times.
Duties:
-Provide personal care to patients in their homes, assisting with activities of daily living such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, pulse, and temperature.
- Assist with mobility and transfers, ensuring patient safety at all times.
- Administer medications as directed by healthcare professionals.
- Assist with meal preparation and feeding, ensuring dietary requirements are met.
- Provide companionship and emotional support to patients.
- Assist with light housekeeping tasks, such as tidying up patient's living space.
Qualifications:
- Previous experience as a Hospice or Home Health Aide or similar role is preferred.
- Knowledge of medical documentation and record keeping procedures.
- Familiarity with memory care techniques for patients with cognitive impairments.
- Ability to follow instructions and work independently with minimal supervision.
- Excellent communication skills, both verbal and written.
- Knowledge of HIPAA regulations and ability to maintain patient confidentiality.
Requirements:
- Certified CHHA
-Maintains a current CPR certification
-Valid driver's license, auto insurance, reliable transportation and willingness to drive
- With or without experience in hospice agency
- Ability to work with patients of all ages
- Strong communication and interpersonal skills
- Compassionate and empathetic approach to patient care
- Active, committed and outgoing personality
- Knowledge of hospice care
