SFC - Health Coach

Health Coach Wanted: Structured Family Caregiver Program.

Overview:

• The Health Coach provides guidance and support to primary family caregivers on the CCSP/SOURCE Structured Family Caregiver program (SFC).

• The Health Coach, in collaboration with the branch RN, provides an assessment of the needs of the SFC caregiver and creates a coaching plan.

• The Health Coach monitors and provides ongoing assessments, training, and support to the primary (family) caregiver and the client, adjusting as necessary.

Responsibilities:

• Provides assessment with the collaboration of the RN to ensure the Family Caregivers’ goals and needs are being met.

• Assist the client and the Primary Family Caregiver with determining eligibility for assistance and resources to complement the SFC program.

• Provides coaching and training to the Primary Family Caregiver to ensure all client goals are being met.

• Maintains effective communication with the Primary Family Caregiver, RN, and Medicaid Waiver Case Manager.

• Monitors the Family Caregivers daily progress notes and tasks performed

• Conducts monthly contact visits with the client and the Primary Family Caregiver by telephone.

• Schedules case conference with the client, the Primary Family Caregiver, the RN, and the Medicaid Waiver Case Manager on a semi-annual basis.

• Sends summary reports to the Medicaid Waiver Case Manager portal monthly, quarterly, and as needed.

Qualifications:

• Must have two years of experience working with elderly, medically frail, and disabled clients.

• Must have excellent communication, computer, and organizational skills.

• Knowledge of GA Medicaid Waiver programs and home and community-based resources is a plus.

• Possesses knowledge of community resources related to medical issues, housing issues, financial and social issues for dealing with clients' care.
