Brief Description
Position Summary
Under the direct supervision of the Medical Director, the Nurse Practitioner is responsible for quality and completeness of assigned medical services at any of the clinic sites. The Nurse Practitioner must be knowledgeable in systems improvement and able to facilitate a Patient Centered Medical Home. The NP provides primary health care services consistent with the philosophy of Rolling Hills Clinic (RHC) and practices under a supervising physician as per California State Nurse Practice Act and facility standardized procedures.

Working Environment: Work is performed in a health clinic setting. The noise level in the work environment is usually low to moderate. Will work with blood-borne pathogens and will require OSHA training. This job routinely uses standard office equipment such as computers, telephones, photocopiers, and fax machines. Provide vaccination records to show proof of immunity for TB, Measles, Mumps, Rubella, Varicella, TDAP & Hep B. If the records are more than 10 years old, titers are required to confirm immunity.

Essential Duties/Responsibilities
- Provide primary care to patients, including histories, exams, lab tests, prescriptions, and referrals within prescribed guidelines and protocols.
- See between 16-20 members per day, ensuring accurate documentation in the electronic health record (EHR) within 72 hours of the visit.
- Perform accurate and timely documentation using Subjective, Objective, Assessment & Plan (SOAP) format; follow RHC, Medicaid, and Medicare guidelines.
- Focus on patients with chronic, unstable, or multi-system problems, as well as acute illness and health maintenance.
- Effectively manage a member panel up to the goal set by the organization.
- Work as part of the medical team, assisting other providers with responsibilities as necessary, e.g., seeing members, processing refills, clearing approval queue, and inbox.
- Assure health education is provided with each encounter when applicable.
- Assist in development of Standards of Care for the medical department.
- Provide members/patients information about evaluation, diagnosis, treatment, prognosis; engage patients in healthcare decisions.
- Provide counseling for diet, hygiene, and preventive healthcare.
- Display the highest level of professionalism, integrity, and a strong work ethic, serving as a model for all clinic staff.
- Maintain requirements of licensure, DEA certification, and board certification (if applicable); fulfill and maintain requirements for credentialing and privileging internally and externally.
- Provide comprehensive, high quality medical services to all members/patients without bias, regardless of age, sex, gender identification, income, national origin or language.
- Consider individuals’ cultural, psychological, social, and environmental factors in creating treatment/care plans.
- Actively participate and engage in quality improvement for clinic efficiency and member quality of care.
- Timely coordination of care among internal/external ancillary providers and hospitals.
- Participate in interdisciplinary activities such as panel management and case conferences.
- Consult supervising physician or colleagues for cases outside scope of practice.
- Participate in QA and CQI activities; attend meetings, clinical audits, peer review.
- Advise Medical Committee/Facility Administration of any situations that may impact medical department efficiency.
- Participate in the development of medical policies/procedures per Tribal policies, accreditation requirements, and professional standards.
- Knowledge of research-based medicine and care; communicable diseases; human nutrition.
- Knowledge of interviewing/counseling techniques; public health/community resources; Indian Health Clinic rules/regulations.
- Ability to establish/maintain working relationships with administrative and peer staff, outside agencies.
- Provide info/guidance regarding RHC personnel and departmental policies.
- Actively participate in meetings/trainings as assigned by supervisor.
- Employee Safety: Safely perform all duties; follow protective protocols to ensure safety.
- Must maintain compliance with ergonomic safety standards; practice regular ergonomic stretches.
- Ensure safety/health for all personnel, including self; take corrective action on safety issues as appropriate.
- Demonstrate cooperative behavior with supervisors, subordinates, colleagues, clients, and the community.
- Perform other duties as assigned.
Require Education & Experience
- Current RN License, Family Nurse Practitioner (FNP) License, and furnishing number
- Certification by either the American Association of Nurse Practitioners (AANP) or the Commission on Collegiate Nursing Education (CCNE)
- Three (3) years of experience working within a community clinic is highly desirable
- Established leadership skills
- Bilingual
- Current DEA license
- Valid BLS Certification
- Valid California Driver’s License
