What are the responsibilities and job description for the Promotor(a) de Salud position at Pascua Yaqui Tribe?
A Promotor(a) de Salud is a trusted member of the community with skills to work with community members and families to promote health and improve quality of life. For the Diabetes Initiative, Promotores(as) will promote health literacy and strive to reduce cases of Diabetes and its complications through education, health coaching, and by coordinating with medical providers and other service providers to connect clients to services.
Skills and Abilities:
- Participate as a member of the integrated care team to coordinate services.
- Conduct assessments which may include environmental, home/safety, and basic vitals .
- Help design or adapt outreach and education materials tailored to the community served.
- Plan, conduct, and/or participate in community workshops and education/outreach events.
- Provide one-on-one education to promote Diabetes and chronic disease self-management.
- Provide lifestyle coaching to individuals and groups.
- Lead or support community activities and events that promote positive lifestyle change among individuals and families.
- Help patients identify and access community services to help address their needs related to social determinants of health such as food security, housing, employment, transportation, and others.
- Participate in required training and continuing education.
- Participate in meetings to plan, coordinate, and assess the activities of Promotores(as).
- Document services provided.
- Submit reports as required.
- Participate in evaluation and quality improvement activities.
- Knowledge of the community and its services.
- Ability to take initiative and work under limited supervision.
- Ability to work closely with colleagues at various agencies in the service system.
- An interest is serving others.
- Appreciation of Yaqui culture, customs, resources and traditions and/or a willingness to learn.
- Sensitivity to community members culture and language.
- Ability to use a computer to document services provided or learn to do so.

- Ability to work a flexible schedule that will include some evening and weekend activities or events.
Skills and Abilities:
- Good communication and interpersonal skills as applied to interaction with co-workers, supervisor, management, Council members, and the public. Have ability to sufficiently exchange or convey information and receive verbal and written work instructions.
- High school diploma or GED preferred.
- At least two years of community service as an employee or as a volunteer is required.
- Must complete CHW (Community Health Worker) Core Competency training within six months of hire.
- Trilingual in Spanish, Yaqui, and English preferred.
- This position will require the incumbent to work non-traditional hours, nights, and weekends;
- Must possess and maintain a valid Arizona Driver’s License;
- Must have a current Level 1 Arizona Clearance Card or be able to obtain the Level 1 Arizona Clearance Card within ninety (90) days of hire.Failure to maintain a current Level 1 Clearance Card will result in termination from this position.
