Responsibilities:

• Provide clinical social work services to patients in a Home Health and Hospice care setting
• Assess patients' mental health needs and develop effective treatment plans
• Collaborate with healthcare professionals to ensure comprehensive care

Requirements:

• Bachelor's degree in social work or a related field
• Licensure as a Social Worker
• Weekend availability and flexibility to work Monday to Friday

Pay: $28.50 - $41.00 / hour
