Responsibilities:
- Provide clinical social work services to patients in a Home Health and Hospice care setting
- Assess patients' mental health needs and develop effective treatment plans

Requirements:
- Bachelor's degree in social work or a related field
- Licensure as a Social Worker
- Weekend availability and flexibility to work Monday to Friday
