What are the responsibilities and job description for the Community Reinvestment Liaison position at Partnership HealthPlan of California?
Overview
In collaboration with departmental and organizational leadership, supports the organization’scommunity reinvestment activities through active and meaningful engagement in identifiedcommunity workgroup and initiatives. On behalf of the health plan identifies and supports, keystrategic activities and interventions that support alignment of collective agency efforts thatpromote and support efforts to encourage member disparity health outcomes.
Responsibilities
▪ Works collaboratively with leaders and community partners in the development of thehealth plan’s community reinvestment plan through active participation in the LocalHealth Department’s Community Health Assessment (CHA) and/or Community HealthImprovement Plan (CHIP) processes and Behavioral Health Transformation plans▪ Helps to develop, track, and monitor the health plan’s shared reinvestment plans foreach assigned county▪ Conducts outreach and engagement to build relationships with stakeholders including,but not limited to: Local Health Departments, hospitals, providers, community-basedorganizations (CBOs), education agencies, grass roots organizations, and other externalpartner agencies.▪ Fosters positive working relationships with providers, members, and/or communitystakeholders to promote organizational objectives and mission.▪ Conducts qualitative research (e.g. Focus Groups, One-on-one Interviews, etc) togather information on identified community gaps, needs and/or available resources.▪ Participates in internal and external workgroups that address health equity and/or healthdisparities▪ Makes recommendations for activities that promote alignment across multi-sectororganizations and agencies▪ Shares and presents qualitative and quantitative data with a variety of stakeholdersincluding, but not limited to: Partnership leadership, Local Health Departments,hospitals, community stakeholders, and/or members.▪ Provides community resources and education at community collaborative meetings andcommunity events as assigned, including education on Partnership benefits, services,updates, wellness programs, and current regulations as applicable in support of improvingthe health and wellbeing of PHC members.▪ Maintains an active working knowledge of regulatory and/or accreditation requirementsrelated to DHCS and NCQA Health Equity Accreditation-related activities and servesas a subject matter expert.▪ Assists the department in completing the Health Equity process for DHCS auditingand/or NCQA survey processes▪ As assigned, supports the health plan’s requirements and activities under applicablememorandum of understanding (MOU) with provider/community partners.▪ Works with organization’s health educators to identify appropriate member educationalmaterials to share with community partners, providers, and members.▪ Performs other duties as required.
Qualifications
Education and Experience
Bachelor’s Degree in Public Health, Community Health or relatedfield. Master’s degree preferred. At least two (2) years of experience inpublic health. Experience in a managed care and/or working withMedi-Cal population preferred.
Special Skills, Licenses and Certifications
Must have excellent verbal and written skills. Experience in workingwith diverse and/or community groups. Experience public speakingand/or presenting materials to a variety of audiences. Valid Californiadriver’s license and proof of current automobile insurance compliantwith Partnership policies are required to operate a vehicle and travel oncompany business. Bilingual skills in Spanish, Tagalog, or Russian arepreferred.

Excellent written, verbal, and interpersonal communication skillsincluding the ability to express oneself clearly and concisely whenproviding service to internal departments, members, providers andoutside entities over the telephone, in person or in writing. Ability tounderstand, analyze, and operationalize regulatory requirements.Ability to analyze and interpret data. Proficiency in community basedprograms and/or county public health services
Work Environment And Physical Demands
Occasional travel required up to 25%. Ability to use a computerkeyboard. Ability to prioritize workload and function effectively withfrequent interruptions and direction from multiple team members.More than 50% of work time is spent in front of a computer monitor.Must be able to lift, move, or carry objects of varying size, weighingup to 10 lbs.

HIRING RANGE:
$ 85,173.51 - $ 106,466.89
