Position Summary

The Behavioral Health Coordinator delivers comprehensive care coordination for children and families referred through the Behavioral Health Hub (BHH), ensuring timely access to behavioral and mental health services. This position actively supports the integration of pediatric behavioral health into primary care settings, collaborating with the Integrated Behavioral Health Director to drive the implementation of team-based, collaborative care models. The coordinator also manages referrals, facilitates data collection, and supports all compliance and reporting requirements essential for sustaining the Behavioral Health Hub.

Responsibilities

· Assist with the facilitation and organization of referrals to support the BHH program among all involved parties for more effective outcomes.

· Provide linkage to community mental health providers, behavioral health professionals, health care networks, and other relevant resources as needed.

· Serve as a point of contact for families and pediatric providers in the referral and access process to behavioral health services, including scheduling, tracking, and follow-up.

· Work closely with BHH Participants (health care providers using BHH services) and Enrollees to support the development and implementation of Behavioral Health Integration Transformation Plans.

· Maintain accurate and HIPAA-compliant medical records for all clients receiving BHH services, including releases and consents.

· Assist in the development of the referral processes from M-BHHs into community behavioral health organizations as needed.

· Collaborate with Behavioral Health Licensed Physician, providers and other team members, offering oversight for direct psychiatric consultation services as appropriate.

· Administer and collect client satisfaction surveys per contract guidelines, using data for continuous quality improvement and reporting.

· Document care coordination services, referrals, training activities, satisfaction survey results, and transition of care coordination in client and program rosters according to quarterly and annual reporting requirements.

· Measure Client satisfaction with BH services delivered by BHH and participants by providing Client Satisfaction Survey at least every 6 months and upon discharge.

· Assure family and youth involvement in all aspects of planning process.

· Provide monthly data for reports as required by the grant. Identify any barriers to success.

· activities.

· Document all Care Coordination Services efforts on the required Client Roster.

· Establish a working relationship with caregiver between the BHH, Consultant, and Enrollee or Participating Provider.

· If Client has care coordination services through another organization, transfer care coordination to that organization if appropriate.

· Assist with identifying, designing, and implementing a BHH quality improvement initiative related to pediatric behavioral health yearly.

· Participate in learning collaborative activities (conference calls, webinars, learning sessions, meetings, etc.) as directed by the Department

· Attend required PCH staff meetings and trainings as appropriate.

· Other duties as assigned

Performance Measures

  • Timely and accurate coordination and documentation of care activities and client referrals per time frames required by the program.
  • Compliance with reporting deadlines, training requirements, and survey collection per contract specifications.
  • Maintenance and submission of client rosters, satisfaction summaries, and transition documentation as required for program audits and deliverable reviews.

Qualifications

· Demonstrated knowledge of healthcare integration, behavioral health service models, and relevant regulations such as HIPAA

· Strong organizational skills with experience maintaining detailed records and compliance documentation.

· Excellent communication and interpersonal skills, including the ability to support clients and families in accessing services.

· Ability to collaborate with multidisciplinary teams, peers and outside organizations.

· Ability to resolve problems, be solution focused and make decisions that enhance organizational effectiveness.

· Organizing: Set priorities, develop task checklists and work schedules, monitor progress towards goals, and track details, data, information and activities.

· Solving Problems: Assess problem situations to identify causes, gather and process relevant information, generate possible solutions, and make recommendations and/or resolve the problem.

Education and Experience

· Licensed mental health professional or healthcare practitioner (e.g., counselor, social worker, nurse, psychologist) with experience in care coordination and pediatric behavioral health preferred.

· A Minimum of a bachelor’s degree in psychology, counseling, child or human services (Registered Clinical Intern or Licensed LMHC or LCSW, preferred)

· 2-4 years screening, assessing mental health disorders and providing cognitive behavioral therapy to justice involved youth.

· 2-4 years’ experience in program development, health care coordination, trauma training, and process implementation, preferably in a pediatric care setting

Work Schedule & Location

Standard hours Monday–Friday, 8:30 a.m.–5:00 p.m. EST, excluding holidays; located at the Jacksonville, FL

Job Type: Full-time

Pay: From $42,000.00 per year
