Responsibilities
- Complete home visits including comprehensive assessments, reassessments, and discharge planning
- Develop and implement individualized patient care plans
- Provide patient and family education and support
- Collaborate with the interdisciplinary team to ensure high-quality outcomes
Qualifications
- Current RN license in the State of Ohio
- Valid driver's license and reliable transportation
- Strong clinical judgment, communication skills, and reliability
- Commitment to ongoing training, CEUs, and required certifications
Pay: $ $80.00 per hour
