Responsibilities
POSITION SUMMARY: The Intake Services Coordinator is responsible for providing assessment, determining clinical need, and make referral to appropriate service within Milestone, Inc.
ESSENTIAL FUNCTIONS:
- Maintain operational knowledge of general nature, goals, and policies of Milestone
- Provide information to professionals, consumers, and families about available services and programs at Milestone, as well as the enrollment process for those services.
- Assist callers in connecting to external resources as needed, ensuring appropriate referrals beyond Milestone services.
- Collaborate with Program Directors and external system representatives to maintain and disseminate current information on program capacities and vacancies, ensuring timely access to Milestone services.
- Ensure completion of all required intake documentation, including all referral forms, authorizations, tracking logs, etc. and completing accurate entry in Electronic Health Record and other relevant systems.
- Support the completion of referral documentation as needed by collecting relevant consumer needs and concerns, and collaborating with other professionals to obtain historical data, prior treatment records, and evaluations.
- Establish and maintain partnerships with community agencies, including Managed Care Organization (MCO), MH/IDA, Office of Children Youth and Families (CYF) and other professionals, to create a reciprocal referral system that allows consumers to access treatment, undergo assessments, and/or be referred to more appropriate services as needed.
- Share referral information promptly with Program Directors and other treatment team members.
- Complete follow up on all referrals and intervene as necessary to address any issues with coordination.
- Coordinate and participate in marketing events and community outreach as a representative of Milestone and its service.
- Respond to all communication in a professional, culturally sensitive, engaging, and timely manner.
- Complete daily insurance verification and ensure that any changes are accurately entered into the Electronic Health Record and communicated to the appropriate program director(s).
- Advocate for all consumers to ensure timely access to natural, community, and specialized services, including providing information, removing barriers, creating options, and resolving issues.
- Attend Partners For Quality (PFQ) Connect support meetings and provide telehealth platform support to staff and clients as needed.
- Perform additional duties as required.
- Manage time effectively and prioritize tasks based on clinical importance and necessity.
- Maintain strict compliance with Milestone Outpatient Department Policies and Procedures, Outpatient licensing requirements, and Partners for Quality Corporate Personnel Policies and Procedures.
- Attend staff meetings and agency training as assigned.

- Excellent communication
