What are the responsibilities and job description for the Home Infusion Per Diem RN - Southern NJ Counties position at Parkway Pharmacy?
Provides direct and indirect patient care services in accordance with the policies and procedures of Parkway Pharmacy, the state’s professional standards and regulatory entities. The Registered Nurse is responsible for establishing and implementing nursing care plan, educating patients/caregivers, provide home infusion nursing services and evaluate progress towards goals.
Ø Perform the initial patient evaluation visit and assess the patient’s physical and emotional status, home environment and safety factors
Ø Provide skilled nursing care to patients and appropriate instructions to patients and their families that include education regarding IV medication, side effects and care and maintenance of IV lines according to Parkway policy, procedure and physicians’ orders.
Ø Educates and counsels the client/family regarding IV therapy procedures and other care needs as appropriate to the client’s condition.
o Utilizes agency educational material as appropriate
Ø Maintains accurate and concise written clinical records for each patient according to agency procedure. Such records must reflect all care given to the patient and all teaching and /or supervision given during each visit. Records must also note any change in the patient’s condition, family or home situation, discharge planning, team conference and communication with physician.
o Submit accurate documentation within 96 hours of visit.
Ø Identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies and drug interactions.
Ø Follows infection control and safety measures in accordance with agency policies and procedures.
Ø Informs the patient’s physician and the clinical supervisor/pharmacist of all pertinent information concerning the patient’s condition and response to treatment, change in status and need for other team involvement.
Ø Cooperate and complies with all pertinent state and local agency regulations governing home health care.
Ø Responsible for following all policies and procedures of the agency regarding service delivery, documentation and care coordination.
Ø Cognizant of problems that may develop during the treatment and therapy. Know the appropriate treatment and act calmly and appropriately in a crisis situation and educate the patient /caregiver accordingly.
Ø Participates in the agency’s quality improvement program.
Ø Participates in discharge planning.
Ø Maintains patient confidentiality per home health care agency policies and procedures.
Ø Maintains good working relationship and open line of communication among appropriate personnel of Parkway.
Ø During Supervisory visits, exhibits full compliance and verbalizes knowledge of agency policies including but not limited to client education, infection control and management of hazardous waste.
Ø Acknowledges and understands that:
o It is policy of the company that field staff is not to utilize the cell phones while driving
o If the employee deems it necessary to call the office to address patient and other issues relating to agency work the employee is to “pull over and park the car” in a safe area (not the side of the road) and make the call.

EDUCATION AND EXPERIENCE:
· Graduate of an approved nursing program.
· Active and unrestricted licensure as a Registered Nurse (RN) in the state of new jersey and other states as required.
· Minimum of 1 year of nursing experience required that contributes to proficiency in infusion therapy (such as medical surgical unit, ER department, ICU, post-anesthesia care unit, or similar setting).
· Excellent communication skills both written and verbal along with strong interpersonal skills.
· Willing and able to work independently in home or alternate – site settings
· Possesses and maintains current CPR/ BLS certifications from American Heart Association.
· Possesses and maintains a current drivers license in good standing.
· Possesses and maintains auto and liability insurance
· Possesses and maintains a car in good working order.
· Experience administering infusion therapy in home or alternate -site setting preferred.
· Must be knowledgeable of the Infusion Nursing Society (INS) standards of practice, policies and procedures.
· Must be knowledgeable of HIPPAA patient privacy and data security requirements, OSHA employee protection regulations, and ACHC standards for home health and home infusion pharmacy (or other relevant accrediting organizations).
· Complies with company policies including employee handbook, quality improvement and safety guidelines.
· Proficient with Microsoft office.

Ø Consistently operates a computer /phone and other office productivity machinery, such as a calculator, copy machine, and computer printer in order to enter , transcribe, record, store or maintain information in written or electronic forms in order to manage patient care.
Ø Regularly lifts, carries, moves and positions objects weighing up to 20 lbs. to transport supplies.
Ø Regularly lifts, moves, and positions patients of various weights from newborn to geriatric.
Ø Continually uses visual and hearing to assess patients.
