What are the responsibilities and job description for the Wraparound Facilitator position at Parkview Health?
Assures consistent care by offering consultation and education to all providers. Monitors progress toward treatment goals. Champions the Children’s Mental Health Wraparound (CMHW) and Children’s Mental Health Initiative (CMHI) program. Ensures that all waiver (CMHW) and DCS (CMHI) related documentation is gathered and reported to the appropriate organizational oversight per requirements. Works in full partnership with team members to develop a revised and annual plan of care (POC). Oversees the implementation of the plan through intentional coordination of care with identified child and family team supports. Facilitates, coordinates, and attends child and family team meetings. Guides the crisis plan development, monitors the implementation, and possibly intervenes during a crisis.
Bachelor’s degree in a human service field.
Must complete FSSA/DMHA-approved training and certification for waiver services. Valid Indiana driver’s license.
Minimum three years of clinical or management experience in human service-related field; and demonstrated two or more years of clinical intervention skills.
Planning and management, quality development and implementation, and healthcare setting experience are strongly preferred.
