Parker Jewish Institute's Certified Home Health Agency is seeking a dedicated and experienced RN-Care Manager to play a key clinical role on our Care Management interdisciplinary team.
Hybrid remote position-this is a telephonic case management position with work from home opportunities!
Parker Jewish Institute's Certified Home Health Agency is seeking an RN, responsible for the psychosocial needs and services of members across the continuum of care. This position will interview, coordinate and plan activities and behavioral routines to meet the social and medical needs of Members and their families. The RN Care Manager will assume a behavioral health leadership role within the interdisciplinary team to achieve optimal clinical and resources outcomes. This is an excellent clinical opportunity for a self-motivated and team oriented Nurse looking to make a difference in the lives of others through coordinating quality care!
Position Qualifications:
- Must be a Licensed RN in New York State
- 2-4 years relevant Nursing and/or Health Care experience with client coordination responsibilities
- Prior experience in a long-term care, managed health or insurance environment preferred
- Certified Case Manager (CCM) a plus
- Bilingual in English and another language a plus: Korean, Mandarin/Cantonese, Spanish, Russian, Creole
- Fully vaccinated against COVID-19
