Job Description:
As a Caregiver/Family Advocacy and Support Services (CFASS) Provider, you will support families and caregivers of children and youth with developmental, physical, or behavioral disabilities by enhancing their ability to care for the child and navigate community services. Your role involves providing valuable techniques and information to help families better understand and address their childs/youth’s needs. You will work closely with the child/youth, their family, and other involved parties to prevent and resolve issues, support family self-sufficiency, and improve the child’s community experience.
Responsibilities May Include:
- Interacting and engaging with family/caregivers and children/youth to offer educational, advocacy, and support resources to develop family/caregivers’ ability to independently access community services and activities.
- Maintaining and encouraging the caregivers’/families’ self-sufficiency in caring for the child/youth in the home and community.
- Addressing the needs and issues of relevance to the caregiver/family unit as the child/youth is supported in the home and community.
- Educating and training the caregiver/family unit on available resources so that they might better support and advocate for the needs of the child and appropriately access needed services.
- Providing guidance in the principles of children’s chronic condition or life-threatening illness.
- Training for the child/youth and/or the family/caregiver regarding methods and behaviors to enable success in the community.
- Directing self-advocacy training in the community with collateral contacts regarding the child/youth’s disability(ies) and needs related to his or her health care issues.
- Self-advocacy training for the child/youth and/or family/caregiver, including during community transitions.
- Providing goal-oriented care to clients as developed in the service plan process.
- Timely documentation of services.
- Adhering to compliance expectations of PSCMH’s Children’s HCBS program.
Minimum Qualifications:
- High School Diploma or Bachelor's Degree
- Human service experience
