Park Nursing Home is actively seeking an RN MDS Nurse / Coordinator for our Skilled Nursing Facility in Rockaway Park, NY.Duties and Responsibilities:Completing accurate assessments, MDS & care plans as assignedInitiating care plans and supporting activities as assignedCreating and distributing monthly care plan calendars in a timely fashionMaintaining & updating all care plans and assessments as requiredMonitoring & auditing clinical records, ensuring accuracy & timelinessInforming DON of persistent issues related to non-compliant documentationProtecting the confidentiality of Resident & Facility information at all timesRequirements:Valid RN LicenseMust be highly organized, professional & motivatedShould have solid computer skillsExcellent communication skillsShould be friendly and a team workerExperience:MDS 3.0: 2 years (Preferred)Long Term Care: 2 years (Preferred )License:NYS RN (Required)
