Position Description
POSITION: FACILITY SERVICES AIDE, Hourly – Non-Exempt
REPORTS TO: Park Lawn Home Manager
DEPARTMENT: Residential – Park Lawn Home
QUALIFICATIONS: 1. High School Diploma or GED; Associate Degree in Human Services preferred
2. One year experience working with persons with Intellectual/Developmental Disabilities
3. One year supervisory experience (preferred)
3. No disqualifying events on criminal background check
4. No disqualifying events on Health Care Workers Registry
5. Pass pre-employment physical, drug and alcohol test
BASIC FUNCTION: Assist Program Manager in supervision of assigned staff and compliance with Park Lawn policies and procedures. Participate in and direct staff in the implementation of the outcomes identified in the Personal Plans. Coordinate shift coverage of DSPs assigned to the Park Lawn Home. Provide role modeling to Direct Support Persons in effective program implementation, support and advocacy of the participants within the program. Drive individuals to medical appointments, shopping, and activities within guidelines and procedures of the Park Lawn Home.
ESSENTIAL FUNCTIONS: THIS POSITION REQUIRES THE EMPLOYEE TO BE ABLE TO SUCCESSFULLY PERFORM THE FOLLOWING:
1. Pass DSP classroom and OJT program approved by Illinois DHS
2. Pass medication training class
3. Pass literacy competency test at 8th grade level
4. Understand written and oral English Language
5. Record observations in participants’ files
6. Write legible, clear, and accurate observations in standard English
7. Lift 50 pounds without assistance
8. Stand, sit, walk, reach, grip, bend and lift as assessed by job analysis screening
9. Remain free of alcohol and/or illegal substance use while working;
10. Become CPR/1st Aid certified and remain certified throughout employment
11. Proficiency in working with basic computer programs (Word, Excel, email, etc.)
POSITION RESPONSIBILITIES:
1. Direct DSP staff in implementation of resident’s Plan including: Recording behavioral observations and participant training data as indicated by the participant's Plan. This includes documentation of programs implemented; documentation and communication of participant physical, social, and emotional needs; documentation of participant behaviors through observation, behavior incident reports and behavior program implementation. Ensure that participants’ Plans and related activities are implemented fully to realize their desired outcomes.
2. Assist in the completion of recreational schedules and programs, while meeting the participant’s individual needs.
3. Receive staff call offs when Manager is not available; arrange for staff coverage consistent with Park Lawn procedures; change personal work schedule to work shifts if coverage cannot be obtained.
4. Maintain a work schedule to include evenings and weekend shifts as directed by the Manager.
5. Coordinate scheduling of and drive participants to medical appointments, shopping, and activities within guidelines and procedures of Park Lawn.
6. Ensure accurate and timely implementation and documentation of emergency drills.
7. Ensure a clean, safe environment of the home. Maintain an effective monitoring system and support to address emergency situations.
8. Administer medications per physician’s order (as indicated on MAR) and document the same in the record.
9. Escort and advocate for participants on medical appointments, including emergencies. Ensure medical paperwork and new orders are forwarded to the RN/T and Manager.
10. Escort participants on community activities, facilitate their full inclusion and act as a representative of Park Lawn. This includes driving facility vehicles to and from activities; ensuring the safety of each participant while on activities; remain responsible and accountable for all monies used during the activity. Stay with participants until relief staff arrives or management staff otherwise instructs you.
11. Ensure resident’s activity money and petty cash is appropriately spent and accounted for in accordance with Park Lawn procedures.
12. Provide other services and assistance to participants, guardians and community as instructed by supervisory and administrative staff. Maintain cordial and professional relationships with resident’s family members and guardian as well other service providers and other Park Lawn personnel
13. Communicate resident incidents, unusual behaviors, and progress toward goals to the Manager.
14. Attend mandatory in-service meetings and trainings.
