Park Gardens Rehabilitation & Nursing Center, has immediate openings for LPNs.  We are looking for a talented and motivated Licensed Practical Nurses (LPN’s) to join our dynamic team. Applicants for this position, should have a passion for geriatric care and be able to work independently without direct supervision. The successful candidate should possess the desire to perform functional therapeutic activities, assist residents improve their physical and medical conditions during their stay at our facility. Shifts Available: Fulltime/part-time/per-diem:

Responsibilities:

·          Assists with the admission, transfer, and discharge of patients.

·          Participates with other team members in planning, implementing, and evaluating nursing care and other patient activities, including diagnostic procedures and treatments.

·          Administers and records medications and treatments as assigned, including IV therapy.

·          Prepares for and assists RNs, nurses, and other healthcare professionals with diagnostic procedures and treatments.

·          Reinforces, supports, and provides healthcare instructions to patients and their families.

·          Communicates observations and reports patients’ vital signs to physicians, NPs, and other healthcare supervisory professionals.

·          Records observations of patients’ responses to care provided in patient charts.

·          Maintains environmental conditions conducive to patients’ comfort and safety.

·          Assists with unit management and/or assumes charge of the unit as assigned.

·          Performs other nursing functions related to patient care and nursing services, as delegated by Nursing Service Administration.

·          Guides and directs other technicians and nurses’ aides in providing patient care.

·          Maintains clinical competence through participation in and attendance at continuing education programs.

·         Assists patients with activities of daily living.

Requirements:

·         Completion of an accredited educational program and current New York State (NYS) licensure as a Licensed Practical Nurse (LPN).

·         Current BLS/CPR certification.

·         Prior experience in a long-term skilled nursing facility is preferred.
