Job Summary
Pareto Clinic is a physician-led family medicine practice in Jacksonville, Florida. We are hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient primary care for adults, women, children, seniors, and families.
This role is ideal for a provider with at least one year of outpatient provider experience as a Nurse Practitioner or Physician Assistant in primary care, family medicine, internal medicine, pediatrics, women’s health, urgent care, or a similar setting.

What This Role Looks Like
This is an outpatient primary care role with no hospital rounding and no OB deliveries. The provider will see patients face-to-face and may also provide telemedicine care.
The goal is not simply to see more patients. The goal is to deliver good care, close charts timely, improve outcomes, support access, and build long-term patient relationships.
This role is best for a provider who appreciates clear expectations, team accountability, continuous improvement, and a practice that is actively building better systems as it grows.
Clinical Responsibilities
- Provide outpatient primary care for adults, women, children, seniors, and families.
- Evaluate and manage acute concerns, chronic conditions, preventive care, and follow-up visits.
- Manage common primary care conditions such as hypertension, diabetes, asthma/COPD, obesity, anxiety/depression, infections, medication refills, and preventive screenings.
- Provide women’s health services within family medicine scope.
- Support pediatric and adolescent primary care, including well-child visits and common acute concerns.
- Review labs, imaging, consult notes, hospital records, and diagnostic results.
- Develop individualized care plans and provide patient education.
- Participate in telemedicine visits and technology-enabled care when appropriate.
- Complete accurate and timely documentation in the electronic health record.
- Collaborate with physicians, providers, medical assistants, care coordinators, referral staff, and quality team members.
- Support care gap closure, HEDIS measures, preventive screenings, risk adjustment, chronic care management, and value-based care initiatives.

Qualifications
- Active Florida APRN or Physician Assistant license.
- At least one year of outpatient provider experience as a Nurse Practitioner or Physician Assistant in primary care, family medicine, internal medicine, pediatrics, women’s health, urgent care, or a comparable setting strongly preferred.
- Comfort caring for adults, women, children, seniors, and families.
- Strong clinical judgment and documentation habits.
- Ability to work collaboratively with physicians, providers, medical assistants, and care team members.
- Commitment to timely chart completion and patient follow-through.
- Professional, dependable, compassionate, and team-oriented.
Preferred Experience
- AthenaOne or Athenahealth experience.
- Telemedicine experience.
- Medicare, Medicaid, HEDIS, quality measures, risk adjustment, chronic care management, or value-based care experience.
- Experience with preventive care, pediatric primary care, women’s health, and chronic disease management.
- Interest in teaching, mentoring, process improvement, or helping shape modern primary care workflows.
Compensation
Full-time compensation ranges from $115,000 to $125,000 annually, depending on outpatient provider experience, schedule, clinical fit, and role responsibilities. Part-time compensation may be offered on an hourly or prorated basis.
Additional opportunities may be available based on quality performance, productivity, teaching, leadership, care coordination, and contribution to practice growth.
Schedule
We are currently hiring for a Monday through Friday outpatient schedule, approximately 3 to 5 days per week, depending on provider availability, practice needs, and clinical fit.
Clinic schedule may include a combination of face-to-face primary care and telemedicine. Specific days, hours, and location will be discussed during the interview.
