Nurse Case Manager - Home Health Care

Paramount Home Health is currently hiring a Full-Time RN.
Monday thru Friday, Daylight shift, reports to one location, No Travel!

The RN will be responsible for (but not limited to):
  • Evaluation and routine skilled nursing visits to our patients
  • Documenting and updating visit notes into Brighttree within 24 hours of visit
- Location of practice limited to one location - Paramount Assisted Living Property

Qualifications:
  • Active Registered Nurse (RN) license Ohio
  • Experience with Oasis preferred, but not required
  • Preferably one (1) year of experience as a Home Care Nurse
  • Has a reliable means of transportation and proof of insurance
